The Partners in Care Foundation are social determinants of health (SDOH) and care transition specialists. The foundation works to align  medical and social care, managing gaps in non-medical care that affect a person’s recovery and overall health—such as food insecurity, medication issues, transportation problems, and home safety. The agency’s HomeMeds℠ is a proprietary, evidence-based identifier of medication-related problems. The Partners in Care Foundation is a leader in training individuals to provide these services for California’s growing aging population.
